Undergraduate Forms

Listed below are all of the forms which may be useful to an undergraduate student.  If you prefer not to submit the form through the website there are PDF files available below of each form that can be printed and turned in to Records and Advisement.  Simply click on the desired form's PDF link and download the file.

 

  • You need to add/drop a class:

Click the link to the PDF version of the form and download the file. Type all the needed information into the required blanks, print out the form, obtain the needed signatures as indicated and return it to Records and Advisement.

  • You and/or your family have moved:

Send and email to addresschange@southern.edu providing your name, ID number, and new address.

 

  • Evaluate your adviser:

Click the below link and fill in all the required blanks on the online form. When you are finished click the submit button to send the form to Records and Advisement.

  • If you need to give Southern Adventist Unversity permission to release your grades to your parents or any other person:

Option 1: Click the below link and complete the permissions section of the webpage.

Option 2: Click the link to the PDF version of the form and download the file. Type all the needed information into all the required blanks, print out the form, sign the form, and return it to Records and Advisement.

  • You can demonstrate your knowledge about a class & a test/examination is available:

Request for Challenge, Waiver, or Validation Examination (PDF)

Note: Please check with the professor of the class first.

 

  • You are a Southern Adventist University student but you need to take a class off campus:

Note the difference in the two below forms and follow the instructions:

Request for Permission to Take Courses Off Campus During Fall and/or Winter Semester (PDF)

Request for Permission to Take Courses Off Campus During the Summer (PDF)

Note: Students wanting to take Home Study courses, need to fill out the Home Study Application and one of the above forms.

 

  • You need a class that is not being offered. An instructor, school/department is willing to teach the class:

Note: After obtaining approval from the department/school chairman/dean, return to the Records and Advisement front desk.

 

  • You need to withdraw from all classes:

Click the link to the PDF version of the form and download the file. Type all the needed information into all the required blanks, print out the form, obtain the needed signatures as indicated, and return it to Records and Advisement.

Drop All Classes (PDF)

 

  • Extenuating circumstances make it impossible for you to complete a class/classes by end of the semester:

Click the link to the PDF version of the form and download the file. Type in all the needed information into all the required blanks in the student section and print out the form.  Then take the form to the professor of the class for him/her to fill out the professor's section.  Finally obtain the approval signature of the department/school chairman/dean and return it to Records and Advisement.

  • You’ve changed your major/minor/degree/professional objective/adviser:

Click the below link and fill in all the required blanks on the online form. When you are finished click the submit button to send the form to Records and Advisement.

  • Your name has changed:

Click the link to the online form and fill in all the required blanks. After completing the form, click the "Submit Form" button to send the form to Records and Advisement for processing.

Request for Change of Legal Name

Note: A copy of the legal document to verify the name change must be sent to the Records and Advisement front desk.  It can be personally brought to the Front Desk, mailed, or faxed to 423.236.1899.

 

  • Exceptions to catalog policy. You have taken a class either at Southern or another accredited college/university that is similar to a general education, major, minor, cognate class you need to fulfill a requirement for graduation at Southern:

  • You are a senior, planning to graduate from Southern Adventist University:

Click the link to the PDF version of the form and download the file. Type all the needed information into all the required blanks, print out the form, obtain the needed signatures as indicated, sign the form and return it to Records and Advisement.

  • You need an official Southern Adventist University transcript:

  • You need to obtain your high school/academy or college/university transcript. Fill out the form for obtaining:

Click the link to the PDF version of the form and download the file. Type all the needed information into the required blanks and print out the form. After signing the form, send the it to the appropriate school for which this request is being made or return it to Records and Advisement.
