Maps and Directions

Health Form
Everyone who participates in one of our adventure programs or challenge course events is required to fill out our health form. Please click the appropriate link below and submit the waiver or print off the .pdf form below and come to your event with it completed. The signature of a parent or guardian is required for minors.

OL Rental Form
If you are renting any outdoor equipment, please fill out the appropriate Rental Form below or if you would prefer to print of the .pdf version you can bring that when you meet to pick up your rental gear. 

Waiver and Release of Liability Form
Everyone who participates in one of our adventure programs or challenge course events is required to fill out our waiver and release of liability form. Please fill out the appropriate digital form or print off the form below and come to your event with it completed.  The signature of a parent or guardian is required for minors.
